The Harlequin Crest is a Unique Item in Diablo 4 that can be worn by any class. I’d even go as far as saying it’s the best helm for certain classes. I use it on my Sorcerer, and it has single-handedly turned my character into an absolute powerhouse that demons fear.
Like other Unique Items, it is better than a Legendary helm and has fixed aspects and perks that cannot be extracted or imprinted. If you’re really lucky, it can be Sacred or Ancestral too. This is a new item variant that has even better stats and features compared to a standard one, but it’s incredibly rare.

Where to find the Harlequin Crest in Diablo 4
The Harlequin Crest is a chance-based drop that can be looted from enemies and chests in World Tier Four: Torment. You’ll need to finish the campaign to unlock World Tier Three: Nightmare and defeat the Fallen Temple Capstone Dungeon in that tier before you can change to World Tier Four: Torment.

Essentially, that means it’s an end-game item, which makes sense when you look at how impressive its stats are. It increases maximum life, resource generation, and stats, reduces cooldowns and damage, and more.
How do you get the Harlequin Crest in Diablo 4?
You’ll need all the luck you can have to get the Harlequin Crest. To increase your chances, I’d recommend spamming Helltide Events, Nightmare Dungeons, or Whispers of the Dead while setting your World Tier to IV.
The Harlequin Crest can be found in Helltide Chests, Resplendent Chests, and Whispering Tree Caches, so doing all three of them is your best bet. Ensure to clear all floors and enemies, and break all the objects you encounter alongside opening all the chests to increase your chances of finding the Harlequin Crest.
If you’d prefer the old fashion way, you can farm Elites and Bosses—both of which have a better chance of dropping it compared to normal enemies. Don’t hold your breath, though. I killed thousands of them and didn’t have any luck.
That said, I have had a lot of fun in the process. Hop from one method to another to keep things fresh. I found myself doing Nightmare Dungeons the most. The thrill of clearing them is highly addictive, and that’s where I ended up finding mine.
